Purpose

The Massive Transfusion Protocol (MTP) is used in the event a patient is undergoing rapid acute blood loss and the bleeding has become uncontrollable and/or life threatening. This protocol streamlines the process of ordering, receiving and dispensing large quantities of blood products. Activation of an MTP should occur when the patient is experiencing severe bleeding such as, but not limited to: blood loss > two liters in two hours, or > three liters overall, >50% blood volume (8-10 RBCs) has been replaced in three hours, or any anticipated ongoing high-volume blood loss.

A blood band is required for this specimen. 

MTP Products

  • 4 units O* packed red blood cells
  • 2-4 units A/AB liquid plasma or FFP
  • 1 platelet pheresis pack

*O positive may be used in adult males over 18 and females over 50

 

Type specific units are not issued based on the patient's history in the computer. The patient's current sample must be typed to give type specific units with an additional confirmation for patients whose type is other than O.

Required Information

Uncrossmatched Transfusion Record
This form is filled out by the blood bank technologist upon activation of the MTP and must include the following information:

  • Date
  • Patient name
  • Medical Record/Hospital Number
  • Uncrossmatched requested time
  • Ready time
  • Technologist signature
  • Unit number of each unit with corresponding unit blood type and expiration date
  • Box Marked at the current status of compatibility testing
  • A check mark indicating RBC or Plasma

A copy is made and kept with the paperwork in blood bank. The original transfusion record goes with the blood products to chart transfusion start/stop times and must be returned to blood bank with an RN's signature at the end of the MTP.

 

Emergency Release Authorization Form
This form is filled out by the Clinician/RN and brought directly to the blood bank window with the following information:

  • Doctor signature
  • RN signature
  • Date/Time requested
  • Blood Bank Bracelet code
  • Patient chart label
  • Box checked for MTP release

The blood bank technologist will complete the bottom portion of the Emergency Release Authorization form after verifying the above requirements.
